WASHINGTON VASCULAR SPECIALISTS
Form privacy notice
How these callback forms work, what they ask for, and what happens in this preview.
How the forms work
These forms allow you to send a general inquiry or appointment request to Washington Vascular Specialists. Information submitted through the forms may be processed by the website hosting environment, email delivery systems, and authorized office staff for the purpose of responding to your request. An appointment request is not a confirmed appointment.
The website may process ordinary technical information needed to deliver and secure pages, such as IP address, browser information, request times, and security logs. Browser settings, extensions, and devices are outside the control of these forms.
Information requested
- Both forms ask for a name, phone number, office preference, optional email address, and an optional provider preference.
- The contact form also asks for a general question topic and offers an optional comments field.
- The appointment form also asks whether you are a new or returning patient and offers an optional preferred date and optional sex selection.
- Both forms ask you to acknowledge permission for follow-up by phone or email and this notice.
Do not enter symptoms, diagnoses, insurance identifiers, payment information, Social Security numbers, or other medical details, including in the comments field. The office can discuss your concerns by phone. The forms do not include file uploads. Providing an email address, comments, preferred date, provider preference, or sex selection is optional.
How your request is used
Submitted details are intended to help authorized office staff respond to your inquiry or discuss appointment scheduling and related practice operations. Staff may follow up using the phone number or optional email address you provide. The contact request delivered to the office may include any comments entered. A scheduling request is not a confirmed appointment. These forms do not authorize marketing messages or text messages.
Email delivery and retention
Form submissions may pass through website hosting and email-delivery systems and may be retained in delivery queues, recipient mailboxes, archives, security logs, or backups. Deleting one email does not necessarily remove every retained copy. Access should be limited to authorized personnel, and no claim of zero storage or guaranteed deletion is made.
Choices and help
Using an online form is optional. To ask a question, schedule care, or raise a privacy concern, call Washington Vascular Specialists at 301-891-2500. If you later ask us not to call about a request, contact the office directly; processing already completed cannot be undone by withdrawing permission.
Urgent concerns
Do not use these forms for emergencies. For a medical emergency, call 911. For assistance with an appointment or general question, call Washington Vascular Specialists at 301-891-2500.
